The VNAHSR is looking for a per diem Wound Ostomy Continence Registered Nurse.
This position is responsible for providing individualized goal directed wound ostomy and continence care through the use of the nursing process.
In addition, this position is responsible for collaborating practice with the physician and other health care team members, providing clinical consultation and direct nursing interventions.
This position is responsible for the provision of education and program development as identified by manager and for fulfillment of contract services as outlined in contracts.

Job DescriptionDevelops, implements, and evaluates programs and systems that facilitate and enhance patient care.
Demonstrates advanced (WOC) clinical nursing skills, knowledge of standards of patient care, clinical and educational consultation, program development and the incorporation of nursing research into nursing practice.
Provides nursing (WOC) services for patients with specific disorders of the gastrointestinal, genitourinary, and integumentary systems this includes wounds, ostomies, tubes fistula, and bowel and bladder incontinence.
Serves as clinical advisor to other in-house and outside agency clinicians on same.
Utilizing the nursing process, assesses patient/care giver needs in order to provide quality care to assigned patients.
Develops and communicates a realistic plan of care for each patient.
Ensures that the patient/care givers are included in the care plan process.
Implements medical plan of care, nursing interventions, and procedures as necessary to ensure the safe effective quality of care for patients.
Evaluates a variety of aspects of implemented plan of care including, but not limited to identified problems, care provided and patient's responses, to meet patient/care giver needs and ensure overall quality of care delivered.
Maintains up-to-date and accurate documentation of nursing care provided to ensure the effective integration of information for use by the health care team to ensure on-going and continued quality of care.
Performs patient and care giver education to ensure informed patient decisions can be made, including, but not limited to, medication side effects, treatment options, discharge instructions, reportable conditions, ranges of services, etc.

RequirementsGraduation from an accredited school of nursing required.
Graduation from a Master's Degree Nursing program.
Graduation from WOC Nursing Educational Program required.
Prior clinical acute care nursing experience required.
Prior Home Care experience preferred.
Ability to obtain valid Vermont State Registered Nurse licensure by date of employment required.
Current Certification from the Wound Ostomy Continence Certification Board.
Ability to successfully pass motor vehicle record check, child abuse, criminal record check and adult abuse check required.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
