Qualifications:
Bachelor's Degree with relevant case management experience Experience with ACT 248 or corrections highly preferred.
Valid Driver's License Availability:
Full time, 40 hrs.
per week.

RequirementsCASE MANAGEMENT SERVICES:
Oversee the person-centered program planning through the supervision, development, and management of the Individualized Service Agreement (ISA)process.
Ensure that all plans are as least restrictive as possible and all aspects of safety are represented within the plans.
Complete necessary documentation by the established deadlines.

Assist clients in the identification of strengths, needs, and goals and create goals that are within Public Safety regulations.
Train staff (possibly in interaction strategies), as requested, in methods and strategies, and monitor for implementation and effectiveness.
Interact with assigned clients on a regular basis.
Review ISA expectations, objectives, and data collection systems to ensure smooth, consistent implementation that services are provided as outlined in the ISA.
Ensure that all regulations are adhered to.
Performs medication administration as assigned.
Participates as a member of the Developmental Services (DS) Emergency On-Call System.
Coordinates care for those clients on Public Safety list, including, but not limited to:
creating safety and behavior plans according to Department of Aging and Independent Living (DAIL) regulations, presenting plans at DS Professional Review, training of staff and home providers or family members on the plans, complete SOTIPS/VOTIPS assessments as required within deadlines, complete all Least Restrictive Environment forms as required, monitor progress on treatment plan goals, and respond to crises for clients on the caseload.
COORDINATION OF HOME PLACEMENT:
Assist Manager in finding homes for clients on caseload and participate in the development of a new home for assigned clients.
Provide training, oversight, and support to the assigned contracted home providers.
Ensure adherence to regulations and the contract.
Review and process monthly documentation.
Visit the home a minimum of one time per month, more often as needed.
Inform home providers of necessary trainings and team meetings to ensure the home provider is part of the team.
PROGRAM COORDINATION/PROGRAM DEVELOPMENT:
Ensure that all waiver services are scheduled and being received by each individual.
In conjunction with the ISA team members, develops individualized services for each individual.
Serves as liaison between agency personnel and others when developing services, especially DAIL Public Safety Specialist.
Ensures goals, objectives, and record keeping systems are developed accurately, written and in place in a timely fashion.
Evaluate individual programs for effectiveness, recommending changes to the Qualified Developmental Disability Professional (QDDP) when needed.
Ensure program expectations are consistently carried out.
Coordinate closely with the DAIL Public Safety Specialist to ensure compliance with all regulations and implement strategies and supports that make safety for all the top priority.
Ensures all court orders and ACT 248 orders are being followed.
Salary Description $24.
